  • Modified Sports (Day 2 of School)
    • Tuesday, September 9 — 3:30 PM | Miller Gym

In our continued effort to make the sports registration process easier for the KCSD families, we have now included the required Interval Health History form to be completed at the time of registration.  This will help ensure that your student is registered and processed in a timely manner. If needed, communication with parents will still be electronic, please be sure the parent's contact information is accurate.

Parents/Guardians: By signing this form, you are acknowledging you have reviewed the questions and provided the medical office with the most up-to-date medical information on your student.

Students: Reminder - you need to be logged on to your KCSD account to register for your sport. Please make sure your parent's contact information is correct for us to communicate what you need in order to be cleared. 

2026 Fall Sports Registration

JV/Varsity Student Athletes Registration Starts: July 24, 2026

Modified Student Athletes Registration Starts: August 9, 2026

Open to students that are in 7th-12th Grade this 2026-2027 school year. 

ALL STUDENTS ARE REQUIRED TO SIGN IN WITH A STUDENT EMAIL ADDRESS.

All athletes must complete the online sports sign-up AND submit medical paperwork to be cleared. Please click the registration link for the school that your student attends below. Students need to be logged on to their KCSD account to register for the sport. When submitting, please make sure the parent/guardian contact information is correct to ensure efficient communication between medical staff and parent/guardians. Once registered, an email will be sent to the parent/guardian email that was provided during registration with more information needed to be medically cleared for the sport.  Once registered, you will need to complete the following steps:

Interval Forms are completed each season. Interval forms will not be accepted before the following dates. Paperwork must be dated on or after the dates list below:

  • JV & Varsity: July 24, 2026
  • Modified: August 9th, 2026

Required forms will be available online via links in the email received after registration AND the KHS Athletic Webpage. Any completed forms can be submitted online to the following email addresses.

JWB Students Grades 7-8: TigerNationBailey@kingstoncityschools.org

Forms can also be picked up at the athlete's school medical office.  Completed paperwork can be dropped off at the athlete's school medical office. If you have questions or concerns, please contact the athletic office at 845.943.3737.

Registration/clearances are done completely online. Once registered, parents will receive an email that was provided during registration regarding their student’s paperwork and if it’s been submitted, reviewed, and cleared. Please check your email for updates on what they may still need to be cleared.  Nurses from each school will communicate via email about any concerns with the student-athletes paperwork and clearance. JV & Varsity Sports Clearance will start in August; Modified Sports Clearances will begin in September.
 

Physical Information

Physicals will be offered at the KHS Medical Office this summer. Please call 845-943-3738 on or after July 13th  between the hours of 7:00am - 12:00pm to schedule for your student. Do not leave a message to make an appointment; Please be sure to speak to someone in the medical office to schedule the appointment.

All student athletes that carry medications, such as inhalers, epi-pens, etc., those that need required immunizations and those that need the Dominic Murray Sudden Cardiac Arrest Form are required to see their own physician. The school physician is not permitted to clear athletes with cardiac issues without written documentation from their primary doctor, nor are they able to write medical prescription for them. Links to these forms are located above.

As a reminder, student-athletes must be registered and medically cleared by the start of the try-out date to participate. Please submit early. Thank you!
